Children who turn 5 on or before November 1, 2012 are eligible for Kindergarten! Children who turn 5 on or before December 2, 2012 are eligible for Transitional Kindergarten! Call the School Readiness Office at 510.317.4759.

Here’s how to register:

1. Make an appointment for a physical with your child’s medical provider.

2. Make an appointment for an oral health exam with your child’s dental health provider.

3. Pick up a registration at your child’s school’s office.

4. Return completed registration packet before June 15th (see schedule attached).

Here is a list of the documents that parents are required to bring at the time of registration. Please see your school’s registration instructions for more complete information.

 Original Birth Certificate

 Parent picture ID (i.e., driver’s license, passport, immigration card, etc.)

 Two (2) current proofs of residence - 1. Rental agreement/mortgage papers 2. PG&E bill, telephone bill, water bill, cable bill PLEASE NOTE: Driver’s license and checkbooks are NOT ACCEPTABLE.

 Report of Health Examination – (on or after March 1, 2012) Written proof of an examination, dated and signed by a doctor.

 Oral Health Assessment - Written proof of an examination - dated and signed by a dentist.

 An ORIGINAL immunization record from the doctor’s office.
